Healthcare professionals strive to provide the best care possible for patients who are seriously ill and support for the families who love them. Because illness is fundamentally personal, and only partly medical, providing optimal care requires seeing and understanding patients as whole persons. National Healthcare Decisions Day (sometimes referred to as NHDD) is an annual initiative celebrated on April 16th to encourage and empower people to begin or continue conversations about their wishes for care through the end of life. NHDD is an initiative to educate people on the importance of advance care planning. It encourages individuals to express their wishes regarding healthcare and for providers and facilities to respect those wishes

1. Identify 4 or more domains of health care quality delineated by the Institute of Medicine (now National Academy of Sciences) in the 2001 Crossing the Quality Chasm report.
2. Identify one or more measures of value (higher quality and/or diminished costs) associated with goal-of-care conversations and in-patient palliative care consultations.
3. Describe a framework for human caring that encompasses individual and family wellbeing through the end of life.
