Overview
Conference is designed to discuss inpatient and acute outpatient cases from WellSpan hospitals and clinics that are concerning for advanced heart failure, and which need expedited review and plan of cares. It provides an opportunity to improve awareness of advanced heart failure throughout WellSpan system and promote timely access to stabilizing therapies. It will also serve to enhance provider education around advanced heart failure among participants and encourage two-way communication regarding patient cases.

Objectives
At the conclusion of this presentation, the participant should be able to:

  1. Identify and discuss best practices for advanced heart failure therapy in inpatient (and acute outpatient) cases from WellSpan hospitals and clinics.
  2. Discuss how to improve awareness and timely access to advanced heart failure therapies throughout the WellSpan system.
  3. Discuss the importance of two-way communication regarding these advanced heart failure cases.
