Overview
This course is designed to showcase the Heart Failure Care Continuum, by discussing the interdisciplinary approaches being taken to provide care for the HF patient.

Objectives
At the conclusion of this presentation, the participant should be able to:

  1. Review pharmacology of medications used to treat heart failure patient in order to design and adjust medication regimens to optimize treatment of heart failure
  2. Identify the concept of palliative care and the importance of introducing these concepts to heart failure patients early in their disease state
  3. Summarize the role of cardiac rehab and how it plays in a role in the improvement of quality of life in the heart failure patient
  4. Discuss effects of sleep apnea on cardiovascular health
