The goals of the Pulmonary Hypertension meetings is to improve multidisciplinary knowledge about patient condition, diagnostic tests and labs, and patient quality of life, and to enhance collaborative multidisciplinary efforts to individualize comprehensive plans of care reducing errors and promote patient outcomes. We will review best practices and formalize our policy, procedure and protocols.

Objectives
At the conclusion of this presentation, the participant should be able to:

  1. Describe pulmonary hypertension program patients
  2. Identify ongoing plans of care to identify opportunities for improvement
  3. Discuss current pulmonary hypertension patient management
