Clinical Documentation Improvement 2020, WellSpan York Hospital

During 2019, opportunities were identified for improvement to risk scoring, publically reported metrics and CMI. Health Information Management has come together with York Hospital leadership to review how accurate documentation positively and negatively impacts those metrics. Data from 3M supports opportunities within the cardiovascular, surgical, and medical service lines for improvement.

1. Discuss publicly reported hospital and physician quality data for hospital medical staff
2. Identify physician documentation practices which facilitate accurate capture of patient severity of illness
3. Describe how specificity of diagnosis is important when documenting clinical conditions
4. Describe how to appropriately apply documentation of post operative conditions, to distinguish between expected clinical entities and complications
5. Integrate improved documentation skills for complete and accurate documentation into more stringent ICD-10 requirements
